Report a Laborer's Death

In the event a Laborer passes away, be prepared to provide the following information when you contact the Trust Fund Office:
  1. Laborer's Name; SSN; date of death
  2. Date of birth, marriage; SSN of surviving spouse or domestic partner
  3. Contact information of closest next of kin or person designated to settle estate
  4. Contact information of person providing notice of death
